I can understand ur problem, but you need to get urself into some kinda of daily exercising routine along with Strict dietary regime.
Eat as much raw veggys and fruits. Add Melons- muskmelon, watermelon, cucumber to your diet
About exercises - join some kinda dance- salsa, merengue, aerobics
or any sports like tennis, baddy etc
Drink only 100 % juices as they dont have extra sugar added. Also for the time being turn to a vegetarian diet and avoid all non vegetarian foods including eggs

But you need more fruits and veggies in and between your meals.
Eat five meals a day, and try to not eat at night. Also, if you could add in a little bit more exercise that would be fantastic! Try loosing weight first, then once you start a good pattern you can start toning up!
If you have a local gym start to go there, swimming helps a lot, and slow jogging, because you can work up to a faster run. Im glad your drinking all that water! (: It will do wonders.
Try cutting out those potato chips though.. go for light popped pop corn or a side salad. And not many fruit juices, if your eating fruits you don't need them.

One of the main problems I can see in your diet is that you eat lots of carbs, such as the chips, cereal, pasta and sandwhich.  I'd only recommend having two of these choices per day as your most likely getting too many...and if you think about it what do they feed animals that are slaughtered for meat?, GRAINS which = Carbs.
